Question for Dr. Salinas: My AST is higher than my ALT (AST 65, ALT 45). My doctor says this could indicate alcohol-related damage but I barely drink. What could cause this?

An AST/ALT ratio greater than 1 can have several explanations besides alcohol. In NAFLD, this reversal can indicate advancing fibrosis — the AST becomes predominant as scar tissue forms. Other causes: muscle damage (AST is also in muscle tissue), cardiac events, or thyroid issues. I would recommend a FibroScan to assess liver stiffness and rule out significant fibrosis. Has your doctor ordered one?
